Yates Elementary
Plan for Success
Dear Parents,
It is with great pleasure that we welcome your child to Yates Elementary. We can all look forward to an exciting and rewarding school year. In order to provide our students with the excellent educational climate they deserve, a School-wide Plan for Success has been developed that will be in effect at all times.
It is in your child's best interest that we work together with regard to his/her education. Your child's teacher will keep you informed about his/her progress in their class, and we ask you to contact them if they can be of any assistance. This plan has already been discussed with your child. We ask your support by reviewing it with him/her before signing and returning this letter. You may keep the Plan to refer to throughout the year.
Sincerely,
Yates Faculty

I have read and discussed the School-wide Plan for Success with my child. I look forward to hearing from his/her teacher about his/her progress. I also will expect to see his/her Student Behavior Record weekly. It will be signed and returned to school on the following day. Together we will celebrate my child's growth in appropriate classroom interaction and behavior.

GOAL OF OUR PLAN
The goal of our plan is to provide a non-threatening environment in which students can become risk takers and reach their fullest academic potential. It will better enable us to instill in each individual an awareness of his/her importance and the need for cooperation among all people in our society. Our plan will allow for quality teacher and student interaction and encourage positive relationships between the school and home. Clear expectations assist students in understanding how to relate in the classroom. When parents and teachers work together, a positive classroom atmosphere is created and academic achievement is increased. As parents, you have expressed a desire for better communication regarding misbehavior before it becomes a serious problem. The staff at Yates agrees and is responding with an
individual Student Behavior Record (SBR). This record will give you the information you need as a parent, on a weekly basis, to assist in behavior intervention. Most importantly, the SBR will encourage everyone to reward students for positive and appropriate behavior. The staff wanted a way to recognize and celebrate with the many students who exhibit proper behavior, and everyone to follow the rules and interact positive at school. We value your involvement as a parent. Together, we make a team!
HOW THE PLAN WILL WORK
The Student Behavior Record (SBR) will be used by all teachers/staff who work with the child. Each child will have his/her own SBR. The SBR will reflect work habits and social development. Specific behaviors needing improvement will be checked. This information will help parents and teachers to communicate and provide appropriate discipline. All SBR's will be sent home weekly for parent signatures. The SBR's will have another section where the students who choose to follow the rules will receive a stamp at the end of each week. Those students who meet the monthly goal will be rewarded.
Specific Guidelines:
1. Students with no write-ups for the week will receive an incentive stamp on their SBR.
2. Students with one or no write-ups in the SBR's will receive a reward from their teacher.
3. SBR's will be sent home weekly, but can be sent home at other times at the teacher's discretion to be viewed and signed by the parent.
4. It is the responsibility of the student to return the SBR weekly, determined by the teacher. If it is not returned, the student will receive a write-up and be ineligible for the classroom reward that week.
5. Students are not allowed under any circumstances to write in, scratch out, or deface the SBR. The SBR is considered an official school document and should such action occur, the student would be subject to a referral to the principal. If an SBR is lost, the student will receive one if available.
6. SBR's will be cleared of all write-ups the first day of each nine weeks. Teachers will, however, keep a record of previous SBR information.
7. Credit earned may be used for incentives.
RULES AND CONSEQUENCES
This plan will help students learn to control their behavior at school. It has three basic rules:
1. Listen and follow directions.
2. Keep hands, feet, and other objects to yourself.
3. Speak respectfully to adults and classmates.
Everyone will follow these rules. If the rules are not followed, there is a specific set of consequences. These rules and consequences are posted in each classroom. Each child knows in advance what will happen if he/she chooses to break a rule. This is the main focus of the plan. Teachers use many positive techniques to help the children make correct choices.
The consequences for breaking rules are:
1. A warning (remind the child of the rule).
2. Spend time in the time out area.
3. A write-up in the Student Behavior Record.
4. Time out away from the classroom.
5. Initiate an Office Visit Request form to the principal/assistant principal
(See attached form for consequences.)
Some severe behaviors cannot be tolerated in school at any time. A child who chooses one of these behaviors will be sent to the office immediately with a district student referral/incident report form. These zero tolerance behaviors (i.e. bullying) are noted in the Hillsborough County Student Handbook.
